For the Normal cookie setting, "Let me decide every time I receive one" and "Accept all cookies" both use the same setting and both accept all cookies except for cookies blocked in the Server Manager. The difference is in how the entries that block cookies get in the Server Manager. The former prompts for you to make a choice and automates the entering. The latter does not and with the latter, if you want entries in the Server Manager to block a cookie, you must manually enter them.
A "3rd party cookie" is defined as a cross-domain cookie. A "Normal cookie" is defined as a cookie set to the server itself. Server to Server cookies on the same domain have no official name, but will be referred to as "cross-server".
If you use the "Let me decide.." setting, choosing certain rules can change your Normal Cookies:/3rd Party Cookies: setting. It doesn't always just add or modify entries in the Server Manager.
If using "Let me decide.." and you don't get prompted, the cookie must have been filtered out by a cookie setting or the server manager, or, Opera automatically accepted the cookie because the cookie settings and the Server Manager allowed it.
